Please bring your doctor's referral with you on your first visit as we must have this in our office before you can be seen by a therapist for treatment. Your initial paperwork and verification of insurance will be handled at your first visit. The initial paperwork can be mailed to you or we have provided patient forms here for you to print, fill out and bring with you. You will need Adobe Acrobat Reader to view them.
